The CHALLENGE Trial Assigned Exercise Across Six Countries
Researchers have long noticed that cancer survivors who stay active tend to fare better, but almost all of that evidence was observational—the kind of study that can show an association but cannot prove exercise was the cause, because people healthy enough to exercise may do better for other reasons. The CHALLENGE trial, run at cancer centers in six countries, removed that doubt the only way science can: it assigned exercise by chance and waited years to see what happened.
889 Patients Coached for Three Years After Chemotherapy
All 889 participants had been treated for stage 3 or high-risk stage 2 colon cancer—disease that had been removed by surgery, followed by chemotherapy, but carried a meaningful risk of returning. Half were randomly assigned to a structured program: regular sessions with a physical-activity coach for three years, aimed at adding roughly the equivalent of a brisk 45-minute walk three to four times a week. The other half received standard health-education materials. Doctors then followed everyone for a median of about eight years, using routine checkups and scans to record whose cancer returned and who remained alive—the long follow-up is what allowed survival, not just fitness, to be measured.
90.3% Alive at Eight Years Versus 83.2%
Five years in, 80.3% of the exercise group remained alive and cancer-free, compared with 73.9% of the comparison group. At eight years, 90.3% of the exercise group was still living versus 83.2%—the 37% lower risk of death. The main downside was modest: muscle and joint complaints, such as strains, occurred in 18.5% of exercisers versus 11.5% of the comparison group.1
Coaching, Not Booklets, Drove the Activity Increase
The results apply to people who had completed treatment and were well enough to take part in an exercise program, and the coaching itself appeared to matter—the booklet-only group increased activity far less. The trial was not designed to explain the biology, though researchers suspect exercise helps regulate insulin (the hormone that controls blood sugar), calms inflammation, and strengthens immune surveillance—the body’s ongoing patrol for abnormal cells. What it did establish is remarkable on its own: a structured exercise program produced survival gains comparable to those of many cancer drugs.
Observational Studies of 90,000 Patients Showed 39% Lower Risk
Large observational studies point the same way: in a pooled analysis of six U.S. studies covering about 90,000 cancer patients, colon cancer survivors doing a moderate amount of activity after diagnosis had a 39% lower risk of death than those doing none.2 A meta-analysis combining 151 patient groups followed over time linked post-diagnosis activity to about a 29% lower risk of dying of colorectal cancer.3 Both track existing habits rather than assigning exercise, so neither shows cause.
