  • Intensive Blood Pressure Treatment Linked to 15% Lower Dementia Risk in a Large Trial

    Adults with untreated high blood pressure who got extra help bringing it down were about 15% less likely to develop dementia over four years than those given usual care, a large randomized trial in rural China found. Because people were randomly assigned to the two approaches, this design offers stronger evidence than a typical observational study that lowering blood pressure may help protect the brain.1

    How the Study Worked

    Researchers enrolled 33,995 adults aged 40 and older who had high blood pressure — a persistent elevation in the force of blood pushing against artery walls — that was not under control. Their 326 villages were randomly split into two groups. In half, trained community health workers helped residents lower their blood pressure with medication and coaching on diet, salt, weight and activity; the other half continued with usual local care. Over four years, the teams tracked new cases of dementia — a lasting loss of memory and thinking ability serious enough to disrupt daily life — using standard memory and daily-function assessments.

    What the Numbers Showed

    The extra support worked: on average, the intervention group’s top (systolic) blood pressure number fell about 22 points more than in usual care. Alongside that, their risk of developing dementia was roughly 15% lower — a difference unlikely to be due to chance. Serious health problems overall were also slightly less common in the treated group.1

    Why Blood Pressure May Matter for the Brain

    Years of high pressure quietly damage the small blood vessels that feed the brain, reducing blood flow and contributing to the tiny areas of injury linked to both vascular dementia and Alzheimer’s disease. Keeping pressure closer to a healthy range may ease that strain, which could help explain why the treated group fared better.

    This was a carefully designed trial, so its results carry real weight — but the program combined medication with lifestyle coaching in one specific setting, and it does not tell any one person which treatment is right for them.
