The Default Bone Advice for Half a Century
Calcium is the mineral that gives bones their hardness, and vitamin D—the nutrient your skin makes in sunlight—is what helps your gut absorb that calcium. Because the two work as a team, taking them in pill form has been standard bone-protection advice for decades, and the supplements remain among the most widely used in the world. The new review, published in BMJ by a team led by Montreal pharmacist Olivier Massé, set out to test whether that advice actually holds up where it counts: in the number of bones people break. The authors concluded that it largely does not, and wrote that clinicians and health agencies should re-evaluate their general recommendations.1
Counting Broken Bones in a Small City’s Worth of People
The review pooled 69 randomized trials—studies in which people were assigned by chance to take calcium, vitamin D, both, or a placebo, so the groups started out alike and any difference at the end can be credited to the pills. Together the trials covered 153,902 adults, roughly the population of a mid-sized city. Most participants (87%) were living at home rather than in care facilities, and nearly three-quarters were not at raised risk of breaking a bone. The trials followed people for about two to three years on average and counted fractures and falls as they actually happened, rather than relying on bone-density scan readings.1
Nine Percent Overall, Sixteen for Hips—Still Under the Bar
Vitamin D alone, tested in 36 trials covering 92,045 people, left fracture rates exactly where they were—no reduction at all, with the evidence rated high certainty. Calcium alone showed about a 9% dip in fractures, but the result was not statistically clear. The combination did better: 9% fewer fractures overall and 16% fewer hip fractures, differences unlikely to be chance. Yet even those reductions fell short of the threshold the review used for a benefit large enough to matter to an individual person, and falls were essentially unchanged under all three strategies. That distinction—statistically real but too small to be meaningful—is the heart of the finding: across a very large body of evidence, the effect that emerged was a modest one.1
Where the Findings Stop
The results describe generally healthy adults, and the review is explicit about who they do not cover: people with osteoporosis (the disease of weakened, fracture-prone bones), people taking bone-strengthening medications, and people with diagnosed vitamin D deficiency, for whom supplementation remains a separate question. There was also too little data to judge residents of care homes or other high-risk groups. Within its scope, though, the finding held steady across subgroups by age, sex, fracture history and dietary calcium intake. It also lands where earlier evidence pointed: in 2018, the US Preventive Services Task Force found the evidence did not support low-dose supplementation for preventing fractures in community-dwelling adults.2
